What to keep ready when you claim

Having the right papers in order is the single biggest reason claims settle quickly. Keep these handy — and call us if anything is unclear.

Health InsuranceDocuments required for a health insurance claim

Cashless claim

  • Pre-authorisation form, filled and signed at the hospital
  • Health insurance card / e-card and policy number
  • Photo ID and address proof of the patient (Aadhaar / PAN)
  • Doctor's consultation papers and advice for admission
  • Investigation and diagnostic reports supporting the treatment

Reimbursement claim

  • Duly filled and signed claim form
  • Original hospital bill with detailed break-up and payment receipts
  • Discharge summary from the hospital
  • Doctor's prescriptions and consultation papers
  • All diagnostic, lab and imaging reports
  • Pharmacy bills with matching prescriptions
  • Copy of policy, photo ID and address proof
  • Cancelled cheque or bank details for NEFT
  • In accident cases: FIR or MLC copy
Motor InsuranceDocuments required for a motor insurance claim

Own-damage claim

  • Duly filled and signed claim form
  • Copy of the vehicle Registration Certificate (RC)
  • Copy of the driving licence of the person driving
  • Copy of the insurance policy
  • Photographs of the damage, taken at the accident spot
  • Repair estimate from the garage
  • Final repair invoice and payment receipt
  • Cancelled cheque or bank details for NEFT

Additional, case by case

  • Third-party damage or injury: FIR copy is compulsory
  • Theft: FIR, original RC, both sets of keys, RTO transfer papers and the police non-traceable report
  • Major accidents: FIR and, where applicable, a police panchanama

Do's for motor insurance

  1. Take a snap of the vehicle at the accident location itself — even if the vehicle has been parked aside.
  2. Register the motor claim within 2–3 days of the accident.
  3. If there is any third-party damage, contact your insurance advisor first.
  4. While purchasing the policy, check the eligible add-ons and coverages.
  5. If you have opted for roadside assistance, take the towing from the company when your vehicle breaks down.

Don'ts for motor insurance

  1. Once you have registered the claim, don't change the reason for the accident.
  2. Don't buy insurance based on the premium alone.
  3. Don't let your policy expire before renewing — a re-inspection will be done and every single damage will be accounted for.
  4. Don't drink and drive. If you do, the claim will not be paid.
  5. Never sign blank proposal forms or written agreements with the insurance company before reviewing your policy.

Do's for health insurance

  1. If you are unwell, first consult a doctor online or at a nearby clinic — and keep the consultation paper safely.
  2. For planned surgeries, take initial approval before admission.
  3. For a reimbursement claim, register the claim before discharge.
  4. For reimbursement, submit documents to the company within 15 days from the date of claim registration.
  5. Understand the policy features, benefits, exclusions and coverage before making a decision.
  6. Purchase your policy through an advisor — you will have support during claims, endorsements and any changes.

Don'ts for health insurance

  1. Don't buy insurance based on the premium alone.
  2. Don't hide pre-existing diseases or any hospitalisation history while taking a policy.
  3. Don't let your policy expire before renewing — the moratorium period will not be completed.
  4. Don't rely solely on employer coverage. Corporate plans usually offer limited cover and end when you leave the job. Always keep a separate, personal policy.
  5. Don't try to take a policy when you already have a planned surgery. Buy the policy before anything arises.
